Nerve-sparing retroperineal lymph node dissection is designed to prospectively identify and isolate the postganglionic sympathetic fibers from L-1 through L-4, prior to doing the lymphadenectomy. Doing so will preserve ejaculation. Both right and left-sided dissections are shown.
