Aorto-esophageal fistual is rare and difficult to repair. Correction requires an understanding of the etiology, diagnosis and management options. Each are discussed in context of a traumatic case associated with a right sided aortic arch. The dramatic endoscopic images, 3D and dynamic cine CT, and aortography are shown.
