Technical improvements allow performance of an aortobifemoral bypass in less than four hours. Specifically, endarterectomy of a circumferentially calcified aorta, end-to-end and end-to-side anastomoses, safe dissection from the iliac bifurcation to the left renal vein, preservation of the IMA and specially designed instruments are shown.
