A 65 year old woman who previously underwent composite graft replacement of the aortic root developed a pseudoaneurysm surrounding the conduit and involving the left main coronary artery. The anatomy was defined by preoperative and Intraoperative studies. The findings and operative repair are presented. An aortogram confirmed completeness of repair.
