Treatment of severe chronic venous insufficiency and venous ulcer by perforator vein interruption using the open Linton approach was flawed by poor wound healing and sepsis. Nevertheless, perforator vein interruption succeeded in preventing recurrent ulcerations for the most part and, when used for treatment of venous ulcers, accelerated healing. Availability of endoscopic instrumentation has allowed the development of the procedure using minimal intervention techniques. This video shows how the procedure can be done using one form of instrumentation. Because surgeons may wish to adapt the procedure to instruments available to them, a background for performing the procedure is given, rationale for including fasciotomy is provided, and indications for the procedure are outlined. What is not described, however, is the marked bettering of the patients and acceleration of ulcer healing which has been experienced by surgeons using the procedure.
