An 81 year old diabetic man with a failed femorotibial in situ vein bypass presented with gangrene of the right great toe. Limb salvage was achieved with a femorotibial bypass to the ankle using polytetrafluoroethylene and a distal vein collar designed to improve prosthetic graft patency.
