A 30 year old male with superior vena caval (SVC) syndrome was found to have mediastinal fibrosis with SVC obstruction. He underwent spiral graft placement from the left innominate vein to the right atrium. This video presents the pre- and postoperative imaging studies and the intraoperative surgical procedure. The patient remains asymptomatic.
