This videotape shows the resection of a right common iliac artery true congenital aneurysm in a four year old female with revascularization of the right lower extremity. The aneurysm was exposed transperitoneally and was completely resected. The right lower extremity was revascularized by mobilizing the left internal iliac artery completely and anastomosing it, end-to-end, to the right external iliac artery, and thus eliminated the need for a venous, arterial or synthetic graft. This videotape shows the gross and histologic picture of the resected aneurysm, clearly depicting all three arterial wall layers in the photomicrograph. The presence of all three layers in the wall of the aneurysm confirmed the diagnosis of an extremely rare true congenital lesion.
