Presented is a 46 year old woman with the recent onset of marked buttock and thigh claudication. The pain is associated with a pulsatile abdominal mass and diminished femoral pulses. Aortography revealed a horseshoe kidney and complete distal aortic occlusion.The video clearly demonstrates the aberrant renal anastomosis, complicating aortoiliac reconstruction.
