The successful management for lymphedema remains a dilemma; fortunately the majority of patients never require surgical treatment. The chronic disabling form may require surgical intervention and despite its potential setbacks, the Charles procedure remains the procedure of choice in most medical centers. We present our modification of the Charles procedure which demonstrates how surgical time and serious complications are minimized and illustrates that return to a productive lifestyle is an achievable goal.
