By definition, a juxtarenal infrarenal abdominal aortic aneurysm is an aneurysm which involves primarily the infrarenal abdominal aorta, but is unique in that it involves the aorta up to, and including, the lower margin of renal artery origin. The peculiarities of this aneurysm consist of difficulty in diagnosing and special method of operative treatment. Due to the fact that the aneurysm projects upward, both from the increased diameter of the aorta as well as elongation, it appears to involve the renal arteries. The diagnosis is made by a certain shadow that is demonstrated in this video, as well as by performing the aortogram in several positions, particularly the oblique position which will show the renal artery arising at the junction of the normal aorta and aneurysm. Treatment consists of graft replacement and the unique method of therapy involves clamping of the aorta at the diaphragm first and then incising the aneurysm, followed by attachment of the graft to the normal aorta with the suture including the lower lip of the renal artery.
