Involvement of the inferior vena cava (IVC) is a controversial risk factor for surgical treatment of adrenocortical carcinoma (ACC).

Herein, we report a case of 47 year old female with ACC with invasion to right kidney, liver and IVC involvement.

She underwent enbloc resection including right nephrectomy, partial hepatectmy and segmental IVC resection and reconstruction using Gortex patch. Patients postoperative course was uneventful. She is alive and cancer free 1 year after the operation.

We believe that that surgical treatment can be effective for management of ACC with extension into the IVC.
