This video demonstrates a surgical modification to the island onlay hypospadias repair. We have found that dissection just below the skin in a plane parallel to the corporal bodies preserves abortive corpora spongiosal tissue that is laterally splayed distal to the hypospadiac meatus. In our experience, incorporation of this abortive spongiosal tissue into an island only urethroplasty diminishes the fistula rate substantially.
