Cure of retroperitoneal sarcomas is predicated on aggressive surgical intervention. This patient was initially felt to be inoperable due to extensive involvement of the cava. Staging revealed no metastases and the giant malignant fibrous histiocytoma was resected in continuity with the right kidney, and the cava from its bifurcation to the renal veins.
