Thoracoscopic Resection of Congenital Pulmonary Cystic Lesion

This is a case of a 2month old male who presented to an Emergency Room with fevers and fussiness. He was born at term and is up to date on vaccinations. A chest X-ray revealed a right cystic pulmonary lesion, which was confirmed on CT scan. The baby was treated with antibiotics for a respiratory infection. Surgery was planned for several months after completing antibiotics course, at 5 months of age. A thoracoscopic right lower lobectomy was performed. The cystic mass was found to be inflammed and adhered to the right upper lobe. The hilar structures were dissected, and divided. The parenchyma of the right upper lobe adhered to the cystic mass was divided. A 12 Fr chest tube was left at the end of surgery. The patient tolerated the procedure well. The chest tube was removed on postoperative day 1. Pathology later confirmed the cystic lesion as a Type 1 Congenital Pulmonary Airway Malformation.
