Laparoscopic Cholecystectomy and Choldedochal Exploration In An Adolescent Patient

A 16 year old male presented with a three-week history of painless jaundice and pruritis. Evaluation revealed cholelithiasis and cho-ledocholithiasis. Preoperative endoscopic retrograde cholangiopancreatography (ERCP) with sphincterotomy was per-formed. However, the common duct stone could not be extracted. The patient underwent laparoscopic cholecystectomy and common bile duct exploration with extraction of the stone the day following the ERCP. A T-tube was placed in the common bile duct for postoperative drainage. He had an uneventful recovery and was discharged on his fourth postoperative day. Specific points to be emphasized include the dissection of an extremely inflamed gall-bladder, laparoscopic exploration of the common bile duct and closure of the choledochostomy with placement of a biliary tube.
