The surgical management of MEN I pancreaticoduodenal neoplasms remains controversial, particularly when ZES is evident. Although distal pancreatectomy, enucleation of neoplasms within the head and excision of duodenal gastrinomas is our preferred procedure, rarely a Whipple procedure as shown in this film may be required.
