Background: Situs inversus totalis (SIT) denotes complete right-left inversion of the thoracic and abdominal viscera. The performance of a laparoscopic pancreaticoduodenectomy in patients with situs inversus totalis is both rare and challenging. Methods:Herein, we report a case of 41-year-old male with situs inversus totalis who presented with a 7-day history of recurrent upper abdominal pain, and founded an tumor in pancreatic head by abdominal enhanced CT scan, who was treated by radical laparoscopic pancreaticoduodenectomy (LPD) .Histological examination of the resected specimen confirmed the diagnosis of pancreatic head cancer. Results: The patient was discharged without any complications on postoperative day 10 after surgery. The patient has been followed up for 6 months and no tumor recurrence or long-term complications were observed. Conclusion: Laparoscopic pancreatoduodenectomy represents a technical challenge for the surgeon, but remains a good option for patients with SIT.
