For patients after completion of neoadjuvant therapy, with encasement but without definitively discernable invasion of the celiac axis or superior mesenteric artery, the "triangle operation" is a unique approach to achieve a complete and radical removal of pancreatic tumor and associated lymphatic or perineural extension along the arterial structures, but preventing morbidity-prone arterial resection and reconstruction. The procedure gets its name from the anatomic triangle bordered by the superior mesenteric artery, celiac axis, and portal vein, which must be completed dissected to ensure a complete oncologic resection. Herein, we underscore the principles of performing this technique robotically and highlight learning points for its effective execution.
