The movie explain the treatment of a patient with a big cystic tumour of the body and tail of the pancreas. The patient had only pain in the left upper abdomen. Laboratory tests were normal. CT Scan demonstrated Cyst of the pancreas with mucine production, occupating almost 60% of the pancreas. No PAAF was taken, markers were in normal levels. Decision for surgery is taken: Total Cyst removal with spleen preservation was proposed. Laparotomy confirmed the diagnosis. Section of gastrocolic ligament with short gastric vessels preservation is done. Tail and body of the pancreas were mobilized. Splenic vein dissected from superior mesenteric vein until the hilium of the spleen. Splenic artery is also dissected. Infiltration of its wall confirmed, and removal of a segment of seven centimetres length produced. Pancreas section was done on the surface of the superior mesenteric vein. TA 45/4.8 was used. Tumour was taken out. Reconstruction of the splenic artery performed by end-to-end anastomosis. Blood flow was normal and Doppler was used. Spleen was normal. Closure of the abdominal wall and opening of the cyst with exploration in side.
