A 43-year-old female, with a BMI of 39.62, with an incidental finding of a 6.2 cm cystic lesion in the pancreatic body. An EUS-FNA reported a hypocellular specimen with mucin presence. The patient underwent selective splenic artery embolization, to decrease the risk of bleeding the same day previous to the robot-assisted distal splenopancreatectomy. The procedure was carried out with challenging dissection as there were multiple adhesions in the area of the celiac trunk, and it was difficult to control the origin of the splenic artery, but it was perform with precision and no intraoperative complications. The operative times was 282min, with and estimated blood loss of 150cc.
