Laparoscopic pancreaticoduodenectomy has been carried out routinely in our center, with more than 300 cases so far. We describe a case of Laparoscopic Duodenum-Preserving Pancreatic Head Resection(LDPPHR) that developed in a 31-year-old woman. The main presenting complaint was epigastric pain. Abdominal computed tomography showed a huge mucinous cystic neoplasm in the pancreatic head. We performed a laparoscopic operation, which resected the total pancreatic head with preserving the duodenum, bile duct, and papilla with end-to-side pancreaticojejunostomy (duct-tomucosa).
