A 49 year old man with chronic pancreatitis presented with a three-yr history of a slowly enlarging epigastric mass that developed after his last episode of acute pancreatitis. CT demonstrated a 15 x 13 x 13 cm pancreatic pseudocyst. We demonstrate open cystogastrostomy and discuss the technical aspects of the procedure in the context of this clinical scenario.
