Two Cases of Islet Cell Tumors: Laparoscopic Approach

This video describes laparoscopic distal pancreatectomy for an insulinoma in an elderly female, with the use of trocars in the paramedian and left flank. A splenic preserving technique is used laparoscopically, and after confirmation of presence of the islet cell tumors, the pancreas is transected using a laparoscopic stapler. Specimen is extracted using a sterile nylon bag through the umbi-licus. Similarly, an islet cell tumor at the junction of tail and body of the pancreas anteriorly are being presented. Using a semi-lateral position, a hook with monopolar cautery to dissect from the nor-mal anterior pancreas parenchyma. Vessels are clipped with titanium clips, and once the islet cell has been completely enucle-ated, it is extracted using the same technique as a distal pancre-atectomy extraction. Both techniques are useful in dealing with benign islet cell tumors of the pancreas located on the left side of the portal vein.
