An 11 year old female presented with a symptomatic islet cell carcinoma of the pancreas with hepatic metastases. To control her symptoms and offer her a potential cure, an en bloc hepatico-pancreatico- gastro-duodenectomy was planned with transplantation. Perioperative diagnostic images, details of the operative procedure, and the postoperative course are shown and discussed.
