Pancreatic necrosis is now recognized as the principle determinant of survival in acute pacreatitis. When present, and if complicated by secondary infection, it invariably proves fatal without surgical intervention. The technique of open drainage has succeeded in reducing mortality rates for infected necrosis to under 20%.
