A 95% pancreatic resection with splenic preservation is performed in an infant with nesidioblastosis. Near-total, rather than 75-80%, pancreatic resection is necessary to prevent recurrent hyperinsulinemia and hypoglycemia. Diagnosis, postoperative management and technical features are briefly discussed.
