We present a video demonstrating parotidectomy for pleomorphic adenoma performed via a limited infra-auricular incision followed by the identification of the buccal branch of the facial nerve distal to the mass. Retrograde dissection of the nerve is then demonstrated. Identification of the parotid duct is then performed. The convergent pattern of the ductules to form the main Stensen Duct is clearly contrasted to the divergent pattern of the facial nerve in our video. This allows for preservation of the facial nerve and of the function of the remaining parotid gland. The branches of the facial nerve are then traced back until free of the mass without needing to identify the main trunk of the nerve. The pleomorphic adenoma is then removed with a cuff of parotid tissue while preserving the facial nerve and uninvolved ductules.
