A new endoscopic technique of fracture repair was employed in an adult patient with an acute subcondylar fracture of the mandible. The endoscope was placed through an intraoral incision. The endoscopic images facilitated fracture dissection and visualization. Anatomic reduction was achieved by endoscopic transbuccal manipulation of the fragments while rigid screw and plate fixation was applied through a transbuccal trocar. This new technique is effective in the treatment of extracapsular subcondylar fractures of the mandible. Fracture imaging was excellent, facial scarring was limited to a cutaneous puncture, and risk of facial nerve transection minimized. Temporary maxillomandibular fixation was applied in-traoperatively and removed prior to the patient's departure from the operating room.
