A massive (20cm x 20cm) basal cell carcinoma of the neck and anterior chest invaded the clavicles and manubrium. En bloc bilateral neck dissections were combined with resection of the manubrium, adjacent clavicles, and ribs 1 - 3. Closure utilized latissimus dorsi transfers; flap necrosis was later salvaged with a pedicled omental graft.
