Three microvascular island transplants from one identical twin were used to reconstruct 75% of her identical twin's scalp, which was lost in an auto accident. The first two islands came from the superficial temporal vascular territory. The third island was donated from the occipital area after preliminary expansion of the scalp. Details of the procedures and complications and their management are outlined.
