The endoscopic endonasal transsphenoidal approach provides superlative visualization of the interface between the normal pituitary gland and a pituitary adenoma. This allows the surgeon to preserve the normal gland, potentially improving endocrine outcomes. Young patients who desire fertility preservation may particularly benefit from this advance. In this video we illustrate the surgical approach and our technique for pituitary gland preservation.
