Single Port (SP) Bilateral TAPP Inguinal Hernia Repair, a Novel Robotic Technique

OBJECTIVE: The DaVinci Single Port (SP) is the newest robotic platform which has the potential to overcome some of the limitations of the previous Single Site (SS) technology. The SP platform is equipped with endowristed and elbowed robotic instruments, which allow greater dexterity in an enclosed space while maintaining access through a single incision. We present a video of a bilateral Transabdominal Preperitoneal (TAPP) inguinal hernia repair utilizing the novel robotic technology. METHODS: A 54-year-old man with hypertension and asthma was evaluated and underwent a bilateral direct inguinal hernia repair using the SP platform. The system was docked through a 2.5cm periumbilical incision, and a TAPP inguinal hernia repair with mesh was completed successfully on both sides. Operative milestone times were recorded throughout the case. RESULTS: The TAPP inguinal hernia repair was successfully completed from skin-to-skin in 91min. Of this time, 10min were spent making the incision and docking the robot, 63min to complete the procedure on the console, and 18min to close the fascia/skin. The patient was discharged from recovery the same day. CONCLUSION: Bilateral TAPP inguinal hernia repair can be performed safely through a single incision utilizing the SP robotic platform. The endowristed and elbowed robotic instruments facilitated dissection and suturing in the narrow spaces. Since the DaVinci SP is a novel technology, further investigation will be necessary to determine the best application of this platform.
