eTEP Access Retroperitoneal Triple NeurectomyThis video shows a step by step eTEP access approach (direct access from lateral to semilunar line) for retroperitoneal triple neurectomy in a patient with severe chronic pain after inguinal hernia repair. Included in the video are pre and postoperative dermatome mappings. The process of selection for this approach is paramount for a successful result. This patient presents with a severe chronic neuropathic pain after an open-mesh, right inguinal hernia repair, four years ago. He underwent multiples procedures, including epididymal cyst removal and a laparoscopic TAPP repair. He does not have a recurrent hernia, orchialgia, nor a meshoma. Multiple pharmacological interventions have failed. He often uses opioids analgesics. His quality of life has been seriously affected.
