This is an educational video demonstrating the technique step-by-step, of posterior component separation with transversus abdominis release for the management of a massive incisional hernia with complete loss of domain. For such, we elected the case of a 29-year old female with a BMI of 41 and a multiple recurrent incisional hernia. The liver, spleen , pancreatic tail and almost the entire small and large bowel were contained in the hernia sac. TAR was performed using the peritoneum to close the posterior sheath and partially reapproximating the anterior fascia. A 50x50cm mesh was used as a sublay for a bridge repair.
