Proper repair of ventral hernias require mesh overlap of the hernia edges in order to avoid recurrence. This is difficult to accomplish when the hernia is bordered by the pubis. In order to provide adequate overlap, a retropubic dissection must be performed in these instances.

A 57 year old female status-post hysterectomy in 2005 underwent laparoscopic ventral hernia repair in February 2006 with placement of mesh. She returned to clinic 6 months later complaining of a low midline buldge. She denied nausea, vomiting and urinary complaints.

We demonstrate the repair of a ventral hernia utilizing retropubic dissection and anchoring in Copper's ligament bilaterally. Retropubic dissection can be performed in order to provide overlap of the hernia defect and minimize recurrence.
