The video depicts our technique (used in greater than 100 cases) of laparoscopic herniorrhaphy using a transabdominal preperitoneal approach for a left indirect inguinal hernia. Important anatomic landmarks such as iliac and epigastric vessels, vas deferens and medial umbilical ligament are identified. Following an incision in the peritoneum and reduction of the hernia sac, a piece of polypropylene mesh is stapled to the transverse abdominus arch, Cooper's ligament and iliopubic tract (which are clearly identified) to cover the entire inguinal floor and effect an anatomic tension-free hernia repair.
