The treatment of large incisional hernias that have "lost their right of domicile" is notorious for its failures. This statement, which is true for incisional abdominal hernias, also applies to lumbar hernias resulting from renoureteral surgery done through a flank incision. Goni Moreno, distinguished surgeon from Argentina, 50 years ago developed a method in which a progressive amount of room air was injected preoperatively into the peritoneal cavity over a period of weeks. In this manner, the patient becomes adjusted to an increased intraabdominal pressure and tolerates the sudden reduction of the viscera during the repair, free of respiratory distress as it occurs with the standard technique. This video demonstrates the application in the repair of a recurrent, large, lumbar hernia following a nephrectomy. Surgical technical details in the correction of incisional hernias are shown in the video which addresses equally to general surgeons and urologists.
