Introduction: since the first report in 1992, published series of laparoscopic approach of liver hydatid cyst have been increasing. Even so, there are certain situations that the laparoscopic approach has not been able to solve, in locations of difficult access , such as posteriors segments of the liver (VII-VII -I ) , the risk of anaphylaxis , spillage of the cyst content , the management of the pericystic layer and the complex cystic-biliary fistulas. The classics basis of liver hydatid cyst surgery can be applied in the laparoscopic approach, providing the benefits of minimally invasive surgery. The objective of this video is to show technical steps of the treatment of hepatic hydatic cyst, applied to the laparoscopic approach. Material And Method: We analyzed the current outcomes with laparoscopic liver approach of hydatid cyst and the technical procedure of cystectomy that apply in patients operated by the same surgical team. Discussion: the basis principles of the liver hydatid cyst open- surgery as: avoid intraoperative spillage ; sterilization of cyst contents and exploration and management of pericystic layer cavity by conservative or radical procedures, can be applied and reproduced by laparoscopic approach. The different series published in recent years show a decrease in mortality, complications and recurrence, which are comparable to the conventional approach, with increasingly lower conversion rates (4.9%) Conclusions: A rational laparoscopic approach of the hepatic hydatic cyst is currently feasible and safe, in centers with human resources trained in HPB laparoscopy and in the treatment of hydatidosis.
