From November 1991 to December 1998, 268 patients with common bile duct stones from 2752 laparoscopic cholecystectomies have been managed laparoscopically. The videotape shows the one stage therapy for common bile duct stones using the laparoscopic approach with the routinely use of intraoperative cholangiography or intraoperative sonography. This study evaluates five ways of laparoscopic management or common bile duct stones: Transcystic duct extraction and a longitudinal incision on the cystic duct until the common bile stones were too big to pass through the cystic opening, primary choledochotomy, antegrade sphincterotomy, and choledochoduodenum anastomosis. This videotape shows the use of all available instrumentation for the laparoscopic approach of common bile duct stones.
