Ileal-duodenal fistula caused by Crohn's disease is not an uncommon phenomenon. Typically, the terminal ileum is the initiating disease segment, although rarely the duodenum may be involved with Crohn's disease, and this may affect the operative approach. An illustrative case of ileal-duodenal fistula is presented along with surgical options and techniques.
