Traumatic avulsion of the nose requires meticulous surgical planning to achieve an optimal functional and aesthetic reconstruction. Herein, we present a 14-year-old male status post dog bite to the face suffering significant soft tissue injuries, including near total loss of the nasal lining, loss of nasal cartilage, and loss of nasal skin. This multimedia presentation highlights key technique principles in the sequence of reconstructive procedures -- early debridement, restoring nasal lining with radial forearm free tissue transfer, structural reconstruction achieved by rib cartilage grafts, and cutaneous coverage provided by a paramedian forehead flap.
