Approaching safely a difficult gallbladder
A case of a 70-year-old female, with a prior history of cholelithiasis, common hepatic duct stenosis, and porcelain gallbladder is presented. She returned to clinic one year after stenting of the CBS, after failing to attend to initial follow-up for removal. A control ERCP demonstrated improved but persistent strictures, and a robotic cholecystectomy was scheduled. During the surgery, the critical view of safety was unable to be attained safely, and a subtotal cholecystectomy was required, with removal of a large impacted stone. With assistance of ICG fluorescence, persistent drainage of bile from the neck of the gallbladder was immediately identified, and the cystic duct orifice was sutured closed from the inside of the gallbladder.
