We present our video that highlights our minimally invasive technique for D2 lymphadenectomy for a total gastrectomy for gastric cancer. This step of the procedure is usually the most difficult and technically challenging part of the entire procedure. It is also a critical step for cancer staging and can be therapeutic in some cases. Our technique utilizes the advantages and aspects inherent to laparoscopy to perform a complete D2 lymphadenectomy where the lymph nodes are kept attached to the gastric specimen. This ensures a single specimen that can be retrieved via a smaller incision and allows the pathologists to appreciate the specimen in its entirety. We present our technique along with tips and tricks to accomplish this successfully and safely.
