Case presentation 65-year-old female with 3 months history of swallowing difficulty, early satiety, nauseas and vomits, sudden weight loss of 10 kg. Physical examination. Her preoperative weight was 48 kg with a BMI of 22 kg/m2, Hb 10.4 g/dL, Albumin 3.4 g/dL. An upper endoscopy showed an ulcerative tumor in the antrum and prepylorus with 80% of lumen obstruction. Biopsies report: stomach adenocarcinoma and signet-ring cells were poorly differentiated. CT scan showed a 3x4cms gastric tumor in the prepylorus area, no liver metastases, and no visible nodes. A laparoscopic approach was planned and a partial gastrectomy was done; the gastric tumor had adhesion to the head of the pancreas, meaning that a surgical separation was necessary. An extended D2 lymph node dissection was performed, as it is shown in the video, her hospital stay lasted 48 hours and the patient had uneventfully recovered.
