We present a case of a 21 year-old male presenting cough and transient sharp chest pain with deep inspiration, a chest xray and subsequent CT scan evidentiated the presence of an esophageal mass. An endoscopic ultrasound was performed, reporting an extrinsic compression at the proximal gastric body. There was a mixed cystic-solid mass located in between the distal esophagus and the thoracic aorta, partially circumferential, measuring 4.6 x 3.6 x 3 cm. A FNA reported tissue consistent with leiomyoma. The video showcases a robot-assisted transhiatal enucleation with a Heller myotomy and Dor Fundoplication.
