Laparoscopic Common Bile Duct Exploration via Choledochotomy with Cholecystectomy

Our patient is an 85 year old male who initially presented to an outside hospital with a one week history of right upper quadrant abdominal pain. He was found to have elevated bilirubin and liver enzymes. A right upper quadrant ultrasound was normal. However, an MRCP demonstrated multiple large stones in the common hepatic and common bile duct. ERCP was not successful for stone removal. Therefore, he underwent a laparoscopic common bile duct exploration via a trans ductal approach. Multiple large stones were successfully removed from the common bile duct using a combination of laparoscopic techniques. The common bile duct was closed with a running suture after a biliary stent was placed. A completion cholangiogram demonstrated no further stones and did not demonstrate a leak. The patient recovered uneventfully. A laparoscopic common bile duct exploration for choledocholithiasis is cost effective, safe, and can achieve a high rate of clearance of stones from the common bile duct.
