Intra-Operative Radiation Therapy (IORT) is a means of partial breast radiation therapy that is part of the armament of radiation therapies offered to women with breast cancer. IORT is an option for women with biopsy proven intraductal carcinomas of the breast with favorable characteristics who have a low likelihood of ipsilateral recurrence. Two studies (ELIOT & TARGIT trials) have validated this technique with acceptable rates of in breast recurrences and lower radiotherapy toxicities. Several centers around the country offer IORT, many women being treated are currently on study to evaluate the oncologic and cosmetic outcomes (CTPR-0009 protocol). IORT was introduces at our center in 2014. Over 75 cases have been completed. Initially training at our center included techniques using retention sutures to ensure an adequate skin bridge. A Quick Close technique was developed to forgo the use of retention sutures and ensure less scaring. The Quick Close technique is completed by placing sutures in the deep tissue to bring the subcutaneous layer together after completing the lumpectomy. A running subcuticular stitch is then started and closed 2/3 of the way prior to placing the balloon catheter. Closing the wound initially allows for a quick closure at the end of the case after the balloon applicator is removed. Cosmetic result is improved and is a much simpler process than the retention suture technique.
