Right sided diverticulitis is a relatively rare entity. We choose to show a case in which laparoscopic segmental colon resection with an intracorporeal anastomosis is performed in an obese patient to treat right sided diverticulitis safely and effectively.

The patient is a 52 year old female with a history of GERD and obesity with a BMI of 39 who presented with a history of recurrent right sided abdominal pain. Imaging revealed multiple right sided colonic diverticula and pericolonic fat stranding consistent with right sided diverticulitis. The patient was treated conservatively initially with bowel rest and antibiotics. The patient returned 6 weeks later for elective resection. A laparoscopic segmental colon resection was performed with an intracorporeal anastomosis. The patient tolerating the procedure well, regained bowel function, and was discharge with an unremarkable postoperative course on postoperative day 4.

The laparoscopic segmental colon resection is performed safely without complication.

Laparoscopic segmental colon resection with intracorporeal anastomosis is a viable option for the treatment of recurrent right sided diverticulitis. The technique of intracorporeal anastomosis can decreased incision size required for extraction in the obese patient.
