Introduction: End stage achalasia is a disabling disease which affects up to 10% of the patients that failed initial surgical treatment. We describe an approach to recurrrent dysphagia after Heller myotomy for achalasia. A case report of a patient successfully treated with a laparoscopic esophagectomy with a description of the steps of the procedure is presented.

Methods: A 42 year old female patient previosly treated with a Heller miotomy for achalasia underwent laparoscopic esophagectomy after failure of surgical and non surgical therapies.

Results: Complete resolution of the symptoms were achieved with a laparoscopic procedure with minimal morbidity and with no complications.

Conclusion: Laparoscopic esophagectomy is a safe and effective treatment for treatment of end stage achalasia.
